1. Acceptance of Terms

By scheduling an appointment or using our services, you agree to these Terms and Conditions. LifeBlood
Mobile Services, LLC may update this document at any time, and the current version will always be available
on our website.

2. Service Areas

We provide mobile phlebotomy services throughout Southwest Florida, specifically in the counties of
Collier, Lee, and Charlotte, serving the area from Naples to North Port.

3. Service Eligibility

Our services are intended primarily for adult patients. At this time, we do not offer pediatric services.

3.1 Scope of Service

LifeBlood Mobile Services, LLC provides blood collection services and transports the specimens to the
patient’s preferred laboratory, including LabCorp or Quest Diagnostics. We do not take responsibility for
the laboratory results or their interpretation.

It is the patient’s responsibility to arrange result interpretation with their healthcare provider. When
specimens are shipped via FedEx or UPS, LifeBlood Mobile Services, LLC is not responsible for any loss,
delay, or issues during transit to the laboratory.

4. Patient Responsibility

Patients must provide accurate and complete information, including: – Contact details. – Relevant medical
history. – Current medications. – Known allergies.

LifeBlood Mobile Services, LLC is not responsible for complications that arise from omitted or incorrect
information.

Patients must also ensure a safe, clean, and appropriate environment for the procedure.

5. Payments

  • LifeBlood Mobile Services, LLC does not accept medical insurance.
  • Payment is direct and must be completed before the service, without exception.
  • An appointment is considered confirmed only after payment is submitted at the time of booking.
  • Once payment is received, a confirmation message will be sent to the patient.

6. Appointments, Rescheduling, and Cancellations

6.1 Appointment Confirmation

Your appointment is officially confirmed once payment has been made.

6.2 Arrival Window

We aim to arrive up to 15 minutes before your scheduled appointment time. However, due to factors such
as traffic or unforeseen delays, we allow a 15‑minute window after the scheduled time for arrival.

6.3 Rescheduling

  • Rescheduling more than 24 hours before the appointment time will not incur any additional fee.
  • If the patient requests to reschedule within 24 hours of the scheduled appointment, a $20 rescheduling fee will apply.

6.4 Cancellations and No‑Show Policy

  • Cancellations made within 24 hours of the scheduled appointment are non‑refundable.
  • If our phlebotomist arrives at the provided address and the patient is unavailable, it will be considered a no‑show, and no refund will be issued.

7. Possible Risks

Phlebotomy procedures may cause minor discomfort such as bruising, mild pain, or dizziness. By booking,
the patient acknowledges and accepts these potential risks.

8. Service Modifications

LifeBlood Mobile Services, LLC may modify pricing, policies, or service availability at any time. Updates will
be posted on our official website.
